Tops Carbonless 2-part Statement Sets

Carbonless, two-part statement forms offer a versatile tool to complete a variety of statements. Each perforated, ruled form includes 14 entry lines, prompts for date, customer information, total amount, and item descriptions with date, charges, credits and balance for each one. Boxes below the item descriptions are labeled "current," "over 30 days," and "over 60 days." Each two-part set includes an original in white and duplicate in canary. Detached size is 5-1/2" x 8-1/2".
